The external parts include a microphone, a speech processor, and a transmitter. The microphone looks like a behind-the-ear hearing aid. It picks up sounds just like a hearing aid microphone does and sends them to the speech processor. The speech processor may be housed with the microphone behind the ear, or it may be a small box-like unit typically worn in a chest pocket. The speech processor is a computer that analyzes and digitizes the sound signals and sends them to a transmitter worn on the head just behind the ear. The transmitter sends the coded signals to an implanted receiver just under the skin.

The internal (implanted) parts include a receiver and electrodes. The receiver is just under the skin behind the ear. The receiver takes the coded electrical signals from the transmitter and delivers them to the array of electrodes that have been surgically inserted in the cochlea. The electrodes stimulate the fibers of the auditory nerve, and sound sensations are perceived.

At your first appointment, an audiologist or speech pathologist will discuss aspects of your / your child’s hearing loss, hearing needs, and the options for improving hearing. This may include; information about cochlear implants and other implantable devices and/or surgeries if appropriate.

Many assessments are required to determine the suitability of an implantable device for each person. The results of these tests allow the clinicians to make an evidence-based recommendation and to advise people on the benefits they may gain from an implantable device compared to conventional hearing aids.

Below is an overview of the types of assessments required to help determine the likelihood of benefit from an implantable device. The assessments will require multiple visits to the Eye and Ear and other service providers over several months.

A cochlear implant is recommended when there is a high chance the person will hear significantly more with the cochlear implant than they currently can with hearing aids.

An implantable bone conduction device may be recommended if there has been a successful trial of a bone conduction device on a headband.

Implantable hearing devices are elective surgeries, and so a recommendation for a device does not mean the person has to proceed with surgery. If surgery is recommended, the final choice to proceed is with the individual patient or a child’s parents.

An audiologist won't turn on (activate) the cochlear implants for about two to six weeks after your surgery — to give the surgery site time to heal.

The fitting of the sound processor occurs approximately two to three weeks following the operation. The sound from a cochlear implant is not like “normal” hearing. It takes time and practice before people become comfortable using the device.

Following the operation, all adults and children attend the clinic regularly for device programming, support, and medical reviews. Patients commit to an ongoing program to help assimilate the cochlear implant into their daily lifestyle. To achieve this, we offer services that aim to optimize patients’ hearing, help develop listening and communication skills, promote speech and language acquisition, and deal with medical issues.

The fitting of the bone conduction implant sound processor occurs approximately six weeks following the operation. There are usually approximately two to three sessions over the first few months following activation.

As the person becomes more proficient in the use of the device and less programming of the sound processor is required, they would only need to visit the clinic every one to two years, or when the need arises.
